Preparation of Supplement Section for University Distinguished Teacher Award

Nominees for this award should first follow the instructions for the Guidelines for Submission for Teaching Awards, then include the following in a separate section behind that.

The supplement should include ONLY information for the three academic years preceding the period covered by the main sections of the submission (i.e., the AORs (like section 2), the Courses (like section 8), and the Course Details (like section 9)).

Additionally, however, this section may also include information on courses taught during the last three academic years—graduate information for those applying for an undergraduate award, or the opposite.

If the nominee does not receive the University Distinguished Teacher Award, the submission automatically will be re-evaluated for a University Teaching Award (if eligible under item 4 of “Eligibility”). The category for the re-evaluation will be based on the nominator, as per the teaching awards.
