Preparation of Binders for Teaching Awards Guidelines

 

NOTE: If guidelines are not followed, binder will not be considered for an award and returned to the department. It may be resubmitted before deadline.

All items listed below should be placed in a three-ring binder (at least 1"—room needs to be available for nomination forms and questionnaires) with the nominee's name and department on the backbone of the binder. Keep the binder as short and concise as possible; separate individual sections below by index tab dividers; do not use plastic sheet protectors. The following items should be included in the binder:

1. Title page: name, department, date of hire at FSU and type of award (Graduate or Undergraduate). The type of student who made the nomination or, if nominated by SUSSAI scores, the level of the class is the determinant on which award the nominee is eligible for (this is noted on the attachment to the memo from the Provost's office/Award Committee).

2. Up-to-date Vitae and Assignment of Responsibilities: highlight items pertaining to teaching. If entering graduate teaching category, include a list of masters/doctoral committees you are on/have been in. Assignment of Responsibilities should include the current one and ones for the past two academic years.

3. Eligibility checklist: must be signed by the departmental chairperson or college dean certifying that the nominee meets eligibility requirements for the award.

4. Syllabus: for each different course taught during Fall 2005 through Summer 2007 (no need for repetition if they are the same each time taught, but please note this fact). If not indicated on the syllabus, provide information on requirements the course fulfills, type of examinations and assignments, and grading policy. Optional material (not to exceed one item per course) may include rationale for course and examples of examinations and/or specific assignments.

5. Enrollment and grade distribution: for each course taught within the designated time period. Use chart that is located at the Provost's Web site.

6. Evaluation evidence: Photocopies of all SPOT or SUSSAI evaluation summaries, or summaries of other evaluation instruments for each course during the designated time period. If the summaries are not included for any course listed, please state reason. Do not include "free response" comments from the evaluations. Candidates who did not have a teaching assignment for one or more semesters during the designated period (Fall 2005 through Summer 2007 for the 2007-08 awards cycle), should include evaluations for the immediately preceding term (Spring or Summer 2005). Fill in the SPOT/SUSSAI matrix that is located at the Web site listed in #5. This matrix includes the median totals for the SUSSAI question #8.

7. Nomination forms. [The Office of the Provost will put in binders upon receipt and they will be returned to nominee after completion of award process, unless requested otherwise.]

8. Statement on Teaching Philosophy. This is a required element. Should not exceed one page. In the teaching philosophy statement, do not duplicate information contained elsewhere in your binder. For example, your syllabus describes what you teach, thus your philosophy statement should focus on why and how you teach.

9. Other supporting materials: (maximum five pages) such as a list of prior teaching awards, description of courses where the nominee is not listed as major instructor, teaching outside FSU, or unsolicited letters from students. No more than a total of three letters may be included. Do not include other supporting letters (i.e., from Chair or Dean, etc.). If applying for an undergraduate teaching award, you may include graduate materials here. Graduate materials do not count in the five page maximum. The same applies for graduate award applicants with undergraduate materials.

10. Questionnaires: this section should have the signed form outlining the distribution of student questionnaires, and should have enough room for the completed student questionnaires (about 30) to be inserted by the Provost's office. The questionnaires must be returned by the student directly to the University Teaching and Advising Awards Committee in care of the Provost's Office (DO NOT HAVE THEM RETURN THEM TO A DEPARTMENT OFFICE). The questionnaire may be mailed, faxed to 850/644-0172, or e-mailed to mvanos@fsu.edu. On receipt by the committee, the questionnaire will be placed in this section of the nominee's binder. The questionnaires will remain confidential and will not be returned to the nominee/department.
